To bring on your first visit:

• A prescription (if needed) for physical therapy from your referring physician, ARNP, or PA. Insurance information (we will make a copy of your card).
• Print out the appropriate patient forms, fill them out, and bring them with you. You can find patient forms here.
• In case of an automobile accident or worker's compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number.
• Please arrive 15 minutes early to complete any remaining paperwork, so you can maximize your time in your evaluation.
• Please wear comfortable clothing that allows easy access to your problem areas.

Each patient's diagnosis is different, so together with your referring physician, your therapist will develop a plan of care that is right for you. Your plan of care and number of visits will be determined during your first visit and explained to you by your physical therapist.

At Bay State Physical Therapy's Wilmington clinic, we provide specialized dance medicine services. Our expert team has successfully guided dancers through recovery from injuries such as Lisfranc fractures and sprains, Os Trigonum syndrome, chondral injuries, tendon strains, tendinosis, psoasitis, and many others. We also offer Pre-Pointe Assessments for those looking to initiate pointe work into their ballet practice.

We offer one-on-one Pre-Pointe Assessments for the adolescent dancer looking to initiate pointe work into their ballet practice. It is recommended that pointe work preparations should begin at least 1 year before the student is fitted for pointe shoes. For this reason, it is recommended that students have their first Pre-Pointe Assessment one year before they plan to start on pointe to assess what areas need to be improved so the dancers can correct all of these essential elements to prevent injury. Pointe readiness assessments are also provided for the advanced dancer looking to return to pointe with skilled guidance following an injury. We also provide one on one general health and wellness screens for dancers in order to identify impairments and limitations, develop individualized injury prevention programs, and can make any necessary referrals to maximize health and performance in dancers.
